These requirements must be submitted annually, irrespective of whether these requirements were completed the previous academic year.

  • Chapter Charter (click link to access the Chapter Charter document)
  • Submit fundraising completed thus far in the academic year (minimum of $50 is due).

Volunteer Requirements
October 31
  • Submit fundraising completed in October (minimum of $150 is due for the academic year to date).
November 30
  • Submit fundraising completed in November (minimum of $250 is due for the academic year to date).
December 30
  • Submit fundraising completed in December (minimum of $350 is due for the academic year to date).
January 31
  • Submit fundraising completed in January (minimum of $450 is due for the academic year to date).
February 28
  • Submit fundraising completed in February (minimum of $550 is due for the academic year to date).
March 30
  • Submit fundraising completed in March (minimum of $650 is due for the academic year to date).
April 30
  • Submit fundraising completed in April (minimum of $750 is due for the academic year to date).

Overview of Program Activities

Program Overview: Teacher training workshops; workshops to train community center staff to connect their clients with free health coverage programs; education programs for students in schools; fundraising.

Training required: Approximately 30 minutes of training includes 8-minute teacher training workshop video, 8-minute community center training workshop video, 5-minute eye health video, 5-minute Fundraising Toolkit Video, and 5-minute video about Unite For Sight’s international programs for which the chapter fundraises.
